Mop sink must be cleaned so that it drains properly. Floors and walls under equipment at floor/wall juncture is very heavily soiled w/food and grease accumulation. this is occurring in the front cook and back kitchen areas. Establish a routine cleaning program that includes verification that these areas are being cleaned. This is a continued repeat violation. Facility is currently experiencing an insect problem which was evidenced by the observation of live and dead insects (ants and roaches) during inspection. Measures must be taken to treat and prevent this issue. Strongly recommend increasing professional pest treatment frequency until problem is controlled. Large gap at bottom of back door. this allows insect/rodent entry. Add weather stripping to eliminate gap.
